Welcome to FBFK’s Estate Planning Portal!
Our goal is to create an easy-to-use and low-cost option for estate planning.
First, create a profile. Second, answer a series of questions. We will use your information to prepare drafts of your estate planning documents. You will be able to review the questions and your answers before you click “submit.” As soon as your answers have been submitted your estate plan drafts will be created and emailed to you in a zip file.
When you receive the drafts, review the documents carefully. If your documents need changes or corrections please let us know. We will edit your documents and prepare them to be signed. You will be contacted by FBFK to schedule a meeting. At our meeting we will review and discuss your documents. We will answer any questions and address any concerns you may have. At the end of the meeting you will sign your documents. We will copy and scan the signed documents, you will leave the meeting with your original signed documents and one copy of each signed document. An electronic copy of each document will be forwarded to you via email.
The documents included in every will package are:
A Last Will and Testament describes how your assets are distributed at your death. In your will you appoint the person or persons who will carry out your wishes (the executor). If you have minor children a will also names the guardian of your children.
A Durable Power of Attorney allows you to appoint specific people to serve as your agent in financial transactions. The power can become effective immediately or only in the event of your disability or incapacity.
A Medical Power of Attorney allows you to appoint specific people to make medical decisions for you if you are unable to speak for yourself.
A HIPAA Release lists the people you authorized to access your private health information. For example, it allows the persons named to speak to your doctors about your treatment history and possible treatment options.
The Directive to Physicians states your treatment preferences when you do not have the ability to communicate and you are suffering from a terminal or irreversible condition that will result in your death if you do not receive that treatment.
